Why Trained ICE Agents Still End Up Shooting Themselves at the Holster

The most dangerous moment in a training day can be the one that looks safest: putting a pistol back in its holster. Internal incident reports provided by a public-records request include a series of training-day accidents within the Immigration and Customs Enforcement agency, including three self-inflicted leg injuries over two days and a different, accidental Taser discharge in an office. Another remarkable similarity in the gun cases is that both shootings were happening as an agent was holstering a duty handgun during quarterly training, a standard practice that has reduced mechanical specifications, body actions and human awareness to a painfully fine tolerance. According to the documents, all of the injured personnel were treated and sent home.

The injuries, in the reports, are not so much the accident of the cinema, but rather as though they were made to happen in accordance with a pre-determined method: a loaded pistol, a closed area of the trigger, a hand that thinks it has reached the “hard part,” and a downward position, where the leg is in the line of fire. According to former senior officials, Newsweek reported that this was not confined to discharge by rookies, and this sentiment is reflected in the descriptions of this incident. An employee was reported to have been hit in the thigh and said that he was “conscious breathing well and in good spirit.” In a second, the teachers stopped the range action as soon as a graze wound that was reported several inches long was reported.

To achieve a clear picture, as to why this one moment continues to reappear, it is helpful to detach language and mechanics. In firearms literature, an unintentional discharge is a shot that was fired at an unintended time: it may be a result of human interaction, equipment malfunction or a mix of the two. This difference is relevant in controlled training conditions, where “nothing should happen,” and where even minor fluctuations, such as finger positioning, holster, clothing belts, or a second of lapse should lead to consequences.

Scott Mechkowski, a retired deputy field office director of the ICE, contended that a bigger federal picture tends to be absent in the responses of the people. He cited a Justice Department study of federal agency shootings between 2000 and 2003 which concluded that about 38% of the reported shooting cases were accidental. Frequently, the qualification of federal officers was also highlighted by Mekowski, as he explained that they were required to complete the test every quarter and a timed pistol course of fire that consisted of 60 rounds with the minimum passing score of 220 out of 260. “Accidental discharges happen,” he said, adding, “Eighty percent of the time, it’s operator error.”

Human factors research assists in understanding the reason why the question of “operator error” is not a satisfactory end. Professor of criminal justice Heidi S. Bonner informed Newsweek that in controlled environments, stress may cause attention to be constrained and fine motor control to be diminished, so the decision-making process is biased toward shortcuts. In the case of holstering, especially, it can turn into a pace task that is overlaying fatigue and repetition and is slightly driven by the urge to stay abreast of the line. Meckowski reported high-tempo details, including long 12-hour shifts, and called “mental fatigue” one of the causes of errors despite the training standards keeping high.

Another layer is that of equipment in certain cases when something is different between the pistol and the belt. An examination of a fatal holster failure by Force Science showed that instead of the holster body, the system had been made unsafe through an additional accessory to add to the holster to increase its strength and retention; during testing, the altered system could be pulled free using approximately three-quarters of the maximum pushing effort of an adult male in less than optimal conditions. The more general engineering argument here is that small interfaces in the aftermarket can push tolerances, leverage and failure points in such a way as cannot be always simulated during training.

Out of the holsters, the handgun itself can become the object of attention when unintentional shots seem to take place without firing any trigger. Some of these models have been recalled or banned in recent years, with agencies reporting misfires and injuries; one popular pistol has received over 100 accidental-discharge reports and 80 injuries since 2016, and some training facilities and departments are reevaluating what they allow on the line. In such cases, the manufacturers tend to blame handling in such disputes and the plaintiffs and investigators examine the design, drop safeguarding conduct, and carry circumstances in the field.

The ICE reports also contain a minor polite reminder, a newly issued Taser fired inside an office when an instructor examined it, and a probe was projected into the carpet, a few seconds. Nobody was hurt, but what the range episodes brought out, was the same message as in the range incidents it is in the everyday dealing that systems and humans collide most frequently.

All through it, the general engineering lesson is that safety on the interfaces: finger and trigger, trigger and holster, holster and belt, policy and fatigue. Once those interfaces are considered as one rather than as disjointed components, the “simple” task of holstering is not an afterthought anymore but instead begins to appear to be the riskiest step on the list.
